Assisted Living
Our Campus offers 3 assisted living homes, Trailview Adult Family Home, Community Group Home and Northview Heights.
Each Assisted Living Provides 24 Hour Staffing, Assistance with Daily Living Skills, Medication Management and Health Monitoring, Nutritious meals and snacks, Housekeeping and Laundry, Recreation and Leisure Activities, and Licensed Nursing Support.

Trailview
Community Group Home
Our Community Group Home is a 4 bed Adult Family Home. Community Group Home is a residence for people who have a brain injury, physical disabilities, mental illness, or cognitive impairment. Our program is designed to support the residents to engage in their surrounding community to promote independence while living at this facility. Our mission is to provide compassionate, individualized care in a home setting.


Northview Heights
Northview Heights is a 20 bed Community-Based Residential Facility that is designed to serve adults with dementia, developmental disabilities, emotional disturbances, mental illness, physical disabilities, traumatic brain injuries or advanced age. Our facility is designed to enhance the quality of life and provide the necessary skills to promote independence. Northview Heights can provide short term stays, long term stays or respite.



Behavioral Health
Clearview Behavioral Health (CBH)
Provides structured rehabilitative services for persons with a variety of diagnoses including mental illness, developmental disability or brain injury who are experiencing difficulty managing within their “home” settings. We offer the security of a locked unit, while providing a homelike environment where individuals receive the services and support necessary for community reintegration. We also offer programming in an unlocked, homelike setting for those hoping to step down to a lesser restrictive environment.
As CBH is a 40-bed State Licensed only facility (not Medicare or Medicaid certified), payment can only be accepted through county contracts, Managed Care Organizations or private funding.
Clearview Behavioral Health also provides comprehensive discharge planning, including inter-agency meetings, trial visits, and an after care plan. From the time of admission through discharge, staff will be working with the individual on placement.
Goals are set to assist the individuals with medication and behavior management, encourage participation in functional living skills and establish routines which promote their successful return into their communities.

Routine meetings are held with the above professionals to assess strengths and needs and evaluate the individual’s progress towards treatment goals. Evaluations will be shared no less than quarterly with involved family members, Managed Care Organizations and county representatives.
The individual’s care and treatment is also monitored by physicians and a psychiatrist. Involved family members and county case workers also complete the team and are an integral part of the transition from CBH to a lesser restrictive setting.

Brain Injury Center
Clearview Brain Injury Center (CBIC)
The Clearview Brain Injury Center (CBIC), in operation since 1991, is a 30 bed state certified, post-acute neuro-rehabilitation program in Juneau, WI. We are located just 45 minutes northeast of Madison, WI and 10 minutes east of Beaver Dam, WI.
CBIC was the first post-acute traumatic brain injury program in the state of Wisconsin. We have offered families and survivors successful outcomes for over 30 years and we continue to be the longest standing program in the State. We work closely with counties throughout the state of Wisconsin through both our admission and discharge process.
CBIC accommodates teens and adults who have experienced a brain injury, impairing their physical functioning and/or cognitive abilities. Our ultimate goal is to help the survivor return to their “home” communities. The Clearview Brain Injury Center utilizes an Interdisciplinary Team Treatment approach to focus on meeting the individual needs of the person through comprehensive assessment. Each plan is individualized to meet the identified expectations and outcomes.

Intellectual Disabilities
Individuals with Intellectual Disabilities (IID)
Clearview has a well established 46 bed household that specializes in person-centered programs specific to the care and treatment of Individuals with Intellectual Disabilities (IID). We are state and federally licensed, providing long term residential placement, short term recuperative care, and respite care. We are trained in the use of strategies and techniques specific to the IID population. The goal of the IID household is to provide outstanding care while striving to achieve the most successful outcomes possible through consistent active treatment.

- Medical Directors on site, on call full time to address medical needs of residents

Nursing & Rehabilitation
Clearview's "person-centered" approach allows us to better serve the people who live here with compassion, dignity, and individuality.
Our households have an open floor plan similar to your own home. Each household has 20 private rooms, a spacious living area and dining room. We provide care for residents with long-term & short-term care needs associated with Alzheimer’s, dementia, rehabilitation and other behavioral health issues in these households.
