Make a Claim

  1. Complete the Unclaimed Funds Form, also in the Treasurer's Office.
  2. Get the claim form notarized. The Dodge County Treasurer's Office does not provide notary services, but the County Clerk's office does. Call 920-386-3600 for availability.
  3. Return completed claim form to the Dodge County Treasurer's Office, Monday-Friday, 8 a.m. to 4:30 p.m.
  4. Provide photo identification (driver's license, passport, state ID card).
