How to Make a Claim

Complete the Form to Claim Unclaimed Funds. Forms are also provided in the Treasurer's Office.

The claim form requires a notarized signature. The Dodge County Treasurer's Office does not provide notary services however the County Clerk's office does. Please call ahead for availability.

Bring your completed claim form to the Dodge County Treasurer's Office,
Monday-Friday, 8 a.m. to 4:30 p.m.

Provide photo identification (driver's license, passport, state ID card).

In accordance with Wis. Stats. 59.66(2)(a)1 “on or before January 10 of every odd-numbered year, each officer of a municipality and county, and each clerk of every court of record, shall file with the treasurer of that person’s county a written report under oath giving the names and the last-known addresses of all persons for whom any such officer or clerk holds money or security, and which has not been claimed for at least one year, and showing the amount of the money or the nature of the security in detail. A duplicate report shall also be mailed to the department of financial institutions.”
