Additional copies of same document furnished at the same time: $3. These fees apply to certified and non-certified copies.

We do not accept out-of-state personal checks. Please pay with a money order.

In-Person Application Requirements

To obtain copies of vital records (birth, marriage, death, divorce (1/1/2016 to date ONLY), domestic partnership or termination of domestic partnership) in person you will need one of these Acceptable Forms of Identification [PDF].

We accept cash, check and money orders for payment in the office. We do accept credit cards, but there is a service charge of 2.65% ($3 minimum). You can also make your request online and pay by credit card.

Through the Mail

To request a copy of a birth, marriage, death, divorce (1/1/2016 to date ONLY), domestic partnership, or termination of domestic partnership certificate through the mail, please print the appropriate application form from the list below and follow the instructions on the application. Mail the application to the address indicated on the form along with a photocopy of the applicant’s current valid photo ID.

Application forms for birth, marriage, death, divorce, domestic partnership, and termination of domestic partnership certificates are available for printing or downloading to your computer.

Military Discharge Records

Veterans are encouraged to take their military discharge papers (DD214) to the local register of deeds for recording. The register of deeds will copy and index the document and give the original back to the veteran. If the original is ever lost, destroyed or damaged, the veteran can always get a certified copy from the register of deeds.

Military discharge papers are valuable to veterans because they may be required to prove eligibility for certain benefits and programs.

If you cannot locate your DD214 military discharge document, contact the Dodge County Veterans Service Office at 920-386-3798 for assistance.

To get your DD214 military discharge document recorded, make sure that it has an original signature. First, take this document to the Veterans Service Office during office hours. Processing will take about 5 to 10 minutes.
