Transition
Our transition program for individuals with disabilities provides support and guidance with the transition into adulthood. We focus on helping young adults gain independence, prepare for employment, pursue further education or training and connect with community resources. Our goal is to empower all individuals to achieve their personal goals and lead a fulfilling, self-directed life.
A benefit specialist can help navigate the system for transitioning individuals starting at 17 years and 6 months of age.

Plan the Possibility
Plan the Possibility is a project designed to promote a positive cultural change of higher expectations among transition-age students and those who interact with them. It includes online social media clips and profiles of transition-age students, an interactive workbook, and the video, "Plan the Possibility."
